That is what orthokeratology does. And for many of the children and adults we fit at Outlook Eye Centre, it genuinely changes how they experience daily life.
Orthokeratology is not new. The technology has been around and improving for decades. But it remains one of the most underappreciated options in optometry, largely because people either have not heard of it or find the concept hard to believe. So let us explain how it actually works, who it is right for, and what to expect.
What is orthokeratology?
Orthokeratology, usually called Ortho-K, involves wearing specially designed rigid gas-permeable contact lenses overnight while you sleep. These lenses are fitted precisely to the shape of your eye and work by gently and temporarily reshaping the front surface of the eye, the cornea, while you sleep.
When you wake up and remove the lenses, the cornea retains the new shape for the day. Light is then focused correctly on the retina, giving clear vision without any correction needed during waking hours.
The effect is temporary and reversible. If you stop wearing the lenses, the cornea gradually returns to its original shape over a few days. This means there is no permanent change to the eye, which is an important point for parents considering this option for their children.

How does reshaping the cornea work?
The cornea is the clear dome-shaped front surface of the eye. In a myopic eye, it is slightly too steep, which causes light to focus in front of the retina rather than on it.
Ortho-K lenses are designed with a specific reverse geometry that applies gentle, controlled pressure to the central cornea during sleep, flattening it slightly. This changes the way light refracts as it enters the eye, moving the focal point back onto the retina where it belongs.
The reshaping happens gradually during the first few nights of wear, with most patients achieving functional daytime vision within one to two weeks of starting. The fitting process is precise and involves detailed measurements of the corneal shape to design lenses that are specific to your eye.
The myopia control benefit
Clear daytime vision is the feature most people come to Ortho-K for. But for children with myopia, the myopia control benefit is just as important, if not more so.
Research has consistently shown that Ortho-K slows the progression of myopia in children. The current evidence suggests a reduction in myopia progression of around 50 percent compared to standard glasses, depending on the study. Axial length growth, the physical elongation of the eyeball that drives myopia progression, is also significantly reduced.
The mechanism behind this is related to the pattern of defocus created by the reshaped cornea on the peripheral retina. This is similar in principle to MiYOSMART lenses, though achieved through a different means.
For children with myopia, particularly those with a strong family history or rapid progression, Ortho-K is one of the most effective tools we have. It delivers two significant benefits at once: freedom from daytime correction and a meaningful slowdown in how fast the prescription worsens.

Who is Ortho-K suitable for?
Ortho-K is suitable for a wider range of people than most expect.
- Children with myopia from around 8 years of age. This is our most common group, and the myopia control benefit makes it particularly valuable for children whose prescriptions are progressing quickly.
- Teenagers and young adults who want freedom from glasses or daytime contact lenses, particularly for sport, performance, or simply preference.
- Adults with low to moderate myopia who are not yet ready for refractive surgery or who want a reversible, non-surgical alternative.
- Active people and athletes. Without glasses or contacts during the day, Ortho-K is popular with swimmers, contact sport players, and anyone whose glasses or contacts are a practical inconvenience.
Ortho-K is most effective for lower to moderate prescriptions. We will assess your prescription and corneal shape at the initial consultation and give you an honest view of whether you are a good candidate before proceeding.
What does the fitting process involve?
Getting fitted for Ortho-K involves more than a standard contact lens fitting. We take detailed measurements of the shape of your cornea using a process called corneal topography, which maps the surface of the eye precisely. These measurements are used to design lenses that are specific to your eye.
The initial fitting appointment is thorough. We will check your eye health, measure your prescription and corneal shape and discuss your goals and expectations.
After the first night of wear, we will review how the reshaping is progressing and check that the lenses are sitting and moving correctly. Most patients need a few visits in the first few weeks as we fine-tune the fit. Once things are settled, review appointments are typically every six months.
What does it feel like to wear Ortho-K lenses?
This is a common question, and an honest one.
Ortho-K lenses are rigid lenses, which feels different to soft contact lenses. Most patients are aware of the lenses when they first put them in, but because they are worn while sleeping, comfort during the day is not a concern. The majority of patients adapt to the sensation of insertion and removal within the first week or two.
Children generally adapt well. In our experience, once they understand what the lenses are doing and start experiencing clear daytime vision, motivation is high. Having a parent who is consistent and supportive with the routine makes a significant difference, particularly in the early days.
Is Ortho-K safe?
Ortho-K has a strong safety record when lenses are properly fitted and patients follow the care instructions carefully. The most important safety consideration with any contact lens wear is hygiene. Thorough hand washing before handling lenses, using the correct solutions, replacing lenses on schedule, and not wearing lenses when eyes are unwell are the fundamentals.

How does Ortho-K compare to other myopia control options?
Ortho-K is one of several myopia control options we offer at Outlook Eye Centre. All of them are effective. The best choice depends on the child, their prescription, their lifestyle, and what the family is comfortable with.
Ortho-K delivers overnight corneal reshaping for clear daytime vision with no correction needed. It tends to suit older children, active children, and those who are motivated by being glasses and contact-lens free during the day.
MiYOSMART lenses are spectacle lenses worn during the day using DIMS technology to slow myopia progression. No contact lens handling is involved, making them a natural starting point for younger children or those not yet ready for contacts.
MiSight contact lenses are soft daily disposable lenses designed specifically for myopia control. A good option for children who want the freedom of a soft lens during the day.
Abiliti contact lenses are another soft daily disposable myopia control option, using a different optical design.
Atropine eye drops use low-dose atropine to slow myopia progression and are typically used in cases of rapid progression or where other options are not suitable, sometimes alongside an optical myopia control treatment.
There is no single right answer. There is only the right answer for your child, at this point in time, and we will help you work that out.
